All laboratory samples received are uniquely identified for tracking purposes and are stored according to the specifications provided by the client.

A Chain of Custody (COC) Record must be completed for all laboratory samples. The original is used for sample tracking throughout the laboratory and remains in its respective job file at all times. When the file is released to the laboratory, a copy is made and retained in the receiving department sequentially by job number.

What to include with sample submission:

• Completed COC
• Material Safety Data Sheet (MSDS), if submitted for the first time
• Certificate of Analysis (COA), if applicable
• Cover letter
